360 Communities has strategically built an infrastructure to collect and report meaningful progress and community impact through data. In addition to reporting output data such as pounds of food distributed, numbers served, and financial information, 360 Communities is now able to show how its holistic approach impacts an individuals movement toward their self-defined goals.
The progress we track aligns with the social determinants of health, which are the areas of stability that individuals and communities need in order to thrive. This includes housing, nutrition, education, and employment, for example. By doing this, we are better able to provide a solid foundation of safety and stability, build trusting relationships with our community, and impact key social determinants of health. This ultimately allows people to envision, plan for, and achieve a brighter future.
